Clinical Observation of Atrial Fibrillation

Abstract
290 cases of atrial fibrillation were abstracted among 9355 adult patients who had been traced ECG at ECG room in HAN YANG UNIVERSITY HOSPITAL from sept. ¢¥74 to sept. ¢¥77.
Author¢¥s clinical observation was focused on the .192 cases who had been traced as in-patient in this hospital.
The results were as follows;
1. The incidence of AlP was 3. 1% in all patients and it was 2. 1% in admitted patients.
2. Sex distribution showed no significant difference and the.majority of patients(65. 7%) were observed over fourth decade.
The average rates of ventricular ¢¥responses to A/F were variable, 58 of patients, had ventricular responses to A/F ranging from 101 to 140 per minute.
4. Associated ECG findings of A/F were L. V. H. (24%), V.P.C. (21.4%), R.V.H. (16.7%), ischemia (9, 4%), L. V. Strain both V. H. (4.7%) and myocardial infarction(3.1%) in the order of frequency 46.4% of patients showed AIF without assoiated findings and 14.1% of patients had A/F-F.
5. Rheumatic valvular H. D. hypertension and arteriosclerosis were the significant underlying diseases of A/F, which occupied 39.6%, 31.8% and 15.1%, of each, respectively. Hyperthyroidism and syphilis were less common underlying diseases, the frequency showing 2. 1% and 0.5% of each, respectively. Undetermined cases occupied 7 M.
6. 67.2% of patients with A/F had complications, such as C. H. F. (45.3%), embolization (19.8%) and SBE (2. 1%). Predilection site of embolization was brain (12%). Other sites were extremity (2.6%), kidney (1. 6%) and mesentery (0.5%)
7. The most frequent underlying diseases of C.H.F. in A/F were rheumatic valvular H. D., hypertension and -arteriosclerosis which included 73 cases out of 87 cases of C. H. F. And predom.inant underlying disease of embolization in A/F was rheumatic valvular H. D. especially M. S. and M. S.I. 24 cases (18.6%) had combined complications.
8. 159 cases among 192 cases were treated with digitalis, quinidine, electric D/C shock and/or combination of them 127 cases out of 142 digitalized patients were effective clinically. But, conversion to R. S. R. by ECG was observed in 12 cases. of digitalized patients, 7 cases of quinidine treated patients and 1 case of ectroversion. 9 cases among these 20 converted patients were readmitted with A/F within one year after discharge.
9. 13 cases of A/F with rheumatic valvular H. D. were examined by echocardiography.
The mean value of echocardiographic left atrial dimension was 47. 1 mm wich was a markedly increased value , compared with a normal value (29mm:1940mm).
